Bremen Clinic:
Every other Thursday afternoon, 1:00-5:00PM

We accept most health insurance plans, including but not to:

• Medicare
• Medicaid
• Blue Cross Blue Shield/Anthem
• Meritain
• CHA
• Humana
• PHCS
• Sagamore

 If your insurance requires a referral please be sure to obtain one from your primary care physician. Co-pays are expected at the time of service.  If a co-pay is not received, a $10 fee will be added to your account.
